<h2>Typical Office ChiroPulse &amp; Table Use:</h2>

<h2>Windows Scaling &amp; Screens</h2>
<p>Windows &#8220;desktop&#8221; does not scale applications like ChiroPulse365 ideally but it does allow for optimization to get ChiroPulse nearly full screen and working very good on Windows 8 tablets while keeping the &#8220;touch points&#8221; large as possible. Setup these optimum touch settings for ChiroPulse:</p>
<ul>
<li>Windows 8 (or 7) Control Panel&#8230; Display&#8230; click Adjust Resolution (on left). Alternately Right-click over desktop and select &#8220;Screen Resolution&#8221; in the popup menu:</li>
<li><strong>Screen Resolution: 1366&#215;768. Click &#8220;Make text and other items larger or smaller&#8221; then select &#8220;Medium 125%&#8221; and hit apply/save/ok. Restart computer or logoff windows.</strong></li>
<li>Control Panel&#8230; power options&#8230; set &#8220;sleep&#8221; to 2-3 hours so that the tablet stays awake until breaks. Once the tablet sleeps ChiroPulse365 must be restarted, a limitation of Windows SQL Server Data Engine.</li>
<li>Start ChiroPulse in &#8220;netbook mode&#8221; from the login window.  Now ChiroPulse will fill essentially the entire &#8220;tablet&#8221; desktop making the touch points as large as possible. You can see a screenshot below&#8230;</li>

<p>Your Office has two options, the first being the best&#8230;</p>
<p>1. Use a Windows 8 tablet (not WindowsRT) and install/license ChiroPulse365 or better yet Dr.Notes to connect the tablet directly to the ChiroPulse Server Database (in your office) via wireless-N. We&#8217;ve been testing with the Acer W510 tablet running Dr.Notes and will test the new Microsoft SurfacePro (not current Surface which runs WindowsRT)  when available in Feb 2013: <a href="http://www.microsoftstore.com/store/msstore/pd/Acer-Iconia-W510-1674-Tablet/productID.258447300/vip.true">http://www.microsoftstore.com/store/msstore/pd/Acer-Iconia-W510-1674-Tablet/productID.258447300/vip.true</a></p>
<p>Some offices hand the tablet to the patients in the treating/exam room however it can be done in the waiting room or wherever you prefer. You can use a capacative stylus (i.e. works with Microsoft Surface or iPad or Android tablet) to make touch screen taps easier (for the elderly not used to using a tablet). Some tablets like the Samsung ATIV SmartPC 500T, ThinkPad2 and upcoming Microsoft SurfacePro have an additional &#8220;digitizer layer&#8221; (under the touch screen) allowing for use with a finer-tip pen which allows for more accurate touch such as handwriting for example.</p>
<p>2. use any 10&#8243; or larger XGA resolution or greater tablet (including the one above) capable or remote desktop (remote login) to connect to an existing CP365 PC on your network. Windows Remote Desktop, GoToMyPC, etc. This is a little clumisier in that the &#8220;touch&#8221; sometimes is less accurate.  No additional license is needed however remoting in from a tablet use would takeover the desktop and screen resolutions could be quirky.</p>
<p>Many additional Windows 8 touchscreen PCs &amp; Tablets will be shipping soon. The Acer W510 features the dual-core energy efficient Atom processor with almost all-day battery life while the Microsoft Surface Pro features a faster quad-core Intel i5 processor with 4-5 hour battery life. It&#8217;s vital to note that <strong>WindowsRT tablets do not support CP365 or any legacy programs, only new windows-8 applets</strong>. Those are useful only for Windows Remote Desktop. Microsoft Surface (running WindowsRT) is exceptional at remote desktop, especially when remoting into a Windows 8 touch pc/laptop.</p>

				<content:encoded><![CDATA[<p>Now available &amp; automated in the December 2012 refresh! You can easily and correctly report PQRS for Medicare (or other carriers). The necessary <strong>CPT G-codes have been added to your system along with logic to prompt for selecting the correct code when posting charges/visit info for  a patient that was designated for PQRS reporting</strong>. This will ensure that you insert the CPT G-codes as needed as well as record the correct ones based on previously used codes.</p>
<h3>PQRS Reporting &amp; Participation Overview (12/26/2012 update, 1/2/2013 update adds two more G-codes)</h3>
<p>You participate in PQRS (Medicare 1/1/2013 requirement) by reporting on the <del>three</del> two(2) measures applicable to DCs on the claim forms (HCFA or EDI claim). There is no registration process for PQRS. The measures are <del><strong>Measure #124</strong> Health Information Technology</del>, <strong>Measure #131</strong> Pain Assessment and Follow-up &amp; <strong>Measure #182</strong> Functional Outcome Assessment. Note that Measure #124 requirement was removed in late December and should not be reported.</p>
<p>To participate you must report each visit on measures #131 and #182. To report you submit a G-code (special CPT code) on your HCFA claim form in box-24. This is done for each visit date for both Measures. If you do NOT successfully report on the PQRS measures in 50% of your claims from 1/1/13 to 12/31/13 you will be PENALIZED 1.5% on all Medicare patients beginning in 2015!</p>
<p><span id="more-1536"></span></p>
<p>Medicare requires 100% compliance for 50% of patients that meet the criteria below. An exception is granted ONLY for patients that fail to meet this criteria:</p>
<ol>
<li>Eighteen (18) years or over</li>
<li>being assessed for pain</li>
<li>using standardized pain assessment tool&#8230;</li>
<li>with documentation of follow-up plan (when pain is present)</li>
</ol>
<p>&nbsp;</p>
<h3>Setup of PQRS is very simple to Report using ChiroPulse365</h3>
<p style="padding-left: 30px;"><strong>Preferred Option “Your Office tab” checkbox added. You must keep this set as the default &#8220;<span style="text-decoration: underline;">neither</span>&#8221; since Measure #124 is not required to be reported at this time by Medicare.</strong> Setting it to “neither” will omit the use of the Measure #124 code.</p>
<p style="padding-left: 30px;"><strong>Claim Wizard Page-1 has a setting to participate (report PORS) for each patient claim.</strong> Enable this for all patients necessary. When checked, the CPT G-codes will be added for each visit. When posting charges the Visit Wizard popup will appear to enter Measure #131 Pain, #182 Functional Assessment &amp; #124 EHR.</p>
<p><strong>The following twelve (12) CPT G-codes have been added in ChiroPulse365 which apply to the three measures for Chiropractic care</strong>.</p>

ChiroServer Tools is used to do your daily primary backup and can also initiating your secondary backup (to secondary device such as USB hard drive or flash drive highly recommended over CD/DVD-R or tape). <p>#<span style="font-size: xx-small;"><strong>4</strong></span> . <strong>Install any ChiroPulse365 refreshes as they become available</strong>.<br />
The login screen will show messages when Refreshes are available &amp; you will be notified by email (see MyOfficeInfo link up top) to obtain the latest version of ChiroPulse365. When installing print/review the Release Notes &amp; changes to learn and familiarize yourself with changes, updates &amp; new features that will benefit your office!</p>
<p>#<span style="font-size: xx-small;"><strong>5</strong></span> . <strong>Setup Wizard &#8211; Work in this for a day or two</strong><br />
Before you enter actual patients or charges you should run the Setup Wizard and work on each step from top to bottom, left to right. This will make your office efficient &amp; avoid pitfalls. For example it is very useful to set your codes, charges, multi-codes and then Fee Schedule Allowed Amounts before you start entering visits/charges.</p>
<p>#<span style="font-size: xx-small;"><strong>6</strong></span> . <strong>Create ChiroPulse username/logins for each staff member</strong>.<br />
You must use the predefined admininstrator login to ChiroPulse365 when you want to create new users. You can login as follows upon starting ChiroPulse365 or change your login from the Utilities&#8230; Login menu choice:</p>

<li>password: left blank until set by the doctor or office managerWe typically recommend you set the Admin&#8217;s password to &#8220;chiropulse&#8221; until you are comfortable using the software but please remember these are encrypted in your database so make a note of the final password once it&#8217;s set. <strong>Note</strong> &#8211; it is best NOT TO USE the &#8220;Administrator&#8221; username for daily use, but to reserve it for maintenance of users.</li>
</ul>
<p>#<span style="font-size: xx-small;"><strong>7</strong></span> . <strong>Setup Your Schedule Office Hours for Each Doctor</strong><br />
Once you enter your Schedule/Office Hours for each treating doctor you can then enter vacations, holidays &amp; other days with special overrides. One tip is to possibly set the the group/clinic to be open every day. This does not effect open slots but allows the yearly view for the clinic/group to show all vacations &amp; special hours days in a very quick glance, without seeing typical days off.</p>
<p>#<span style="font-size: xx-small;"><strong>8</strong></span> . <strong>Electronic Claims Submission &amp; Setup</strong><br />
If you have an Clearinghouse from previous software contact them and submit some sample HCFA Print images. Please review our: <strong><a href="http://www.chiropulse.com/center/showthread.php?t=207">Electronic Claims Submission: start to finish document &#8211; click here</a></strong></p>
<p>At minimum you should immediately erify &amp; setup your EDI Electronic Claims Folder. In ChiroPulse365 click the top UTILITIES menu&#8230; &#8220;EDI Folders &amp; Paths&#8221; then click the <strong>reset </strong>button. This will properly set the edi path to your server or stand-alone PC.</p>
<p>#<span style="font-size: xx-small;"><strong>9</strong></span> . <strong>HCFA-1500 Forms</strong><br />
ChiroPulse365 has visual indicators wherever HCFA related data is entered. This will rapidly allow you to relate &#8220;HCFA Form&#8221; data to ChiroPulse365 and more importantly allow you to efficiently enter data that might be reused in later patient claims (future treatment in your office). In addition use the <strong>HCFA Helper</strong> window in MyPatients and links to our HCFA-1500 tutorial.</p>

<p>#<span style="font-size: xx-small;"><strong>11</strong></span> . <strong>Create New Patients for active/treating patients</strong> as they present in your office. This can be done directly from scheduler or PatientTab/finder. When entering new patients it is important to complete the <strong>new patient checklist</strong> so that you can quickly post charges, create valid HCFA/invoices (with proper ICDs) as well as use correct billing codes and/or fee schedules (contracted rate allowed amounts). For patients with <strong>insurance</strong> verify their policy &amp; enter the insurance timeframe data in complete so that patient portions will be automatically calculated when invoicing.</p>
<p>#<span style="font-size: xx-small;"><strong>12</strong></span> . <strong>Don&#8217;t forget to run your backup from ChiroServer Tools</strong>. Every day the office is open, without exception.</p>
<p>#<span style="font-size: xx-small;"><strong>13</strong></span> . <strong>Follow-up with insurers and/or your clearinghouses to obtain your PINs, NPI &amp; other billing IDs</strong> required when submitting to local insurance carriers. Determine if you will invoice from the treating doctor or from the Office/Group/Clinic. Delay or failure to obtain PIN/NPIs/etc. will put a strain on your short term collections as you await approval and paperwork to clear.</p>
<p>#<span style="font-size: xx-small;"><strong>14</strong></span> . <strong>When invoicing you might want to do a few patients individually to get a feel for how your office will invoice &amp; submit electronic claims</strong>. Use &#8220;bill many&#8221; when you are comfortable and after you successfully submitted electronic claims EDI files. Experiment with the Preferred Options Invoice Options to get the Patient Invoices working best for your office!</p>
<p>#<span style="font-size: xx-small;"><strong>15</strong></span> . <strong>Communicate with your Staff</strong>. Each office worker can have a useful role without tripping over each other. Use Todo notes in ChiroPulse365 for doctors &amp; staff.</p>

The Patient Tab End of Day window shows you some of the more popular reports or things to do. Consult with the doctor or office manager as to what is expected to be done.</p>
<p><span style="text-decoration: underline;"><strong>Backup your ChiroPulse365 Database</strong></span><br />
IMPORTANT: At the end of the day it is important to BACKUP your database. From MyOffice tab click &#8220;HOW TO BACKUP&#8221; for an overview. When you are ready Exit ChiroPulse365 (on all PCs) then go to your SERVER PC and open ChiroServer Tools. Run the Daily Backup &amp; Secondary Backup for database <strong>ChiroPulsedbMaster</strong>. This will save a backup of your database to the servers backup folder and copy it to a removable drive for added protection.</p>
<p><span style="text-decoration: underline;"><strong>Scheduler &amp; Posting Charges</strong></span><br />
Most offices use the Scheduler and this is where you can see upcoming appointments, enter followups &amp; post charges quickly. you can change appointment status&#8217; from pending to waiting to treating to kept, etc. Once you mark Kept that will post any associated codes/charges to the Ledger, as well as be prompted to enter copays/payments &amp; print walkouts. You can edit an appointment (while &#8220;treating&#8221; or &#8220;waiting for example&#8221;) to add or change any CODES/CHARGES prior to marking Kept. Use the &#8220;Add visit/charges&#8230;&#8221; menu choices to post additional charges after marking Kept. Use Ledger to edit or delete posted payments or charges.</p>
<p><span style="text-decoration: underline;"><strong>Electronic Claims vs. Paper Claims</strong></span><br />
In MyOffice Tab&#8230; Preferred Options icon you will see the <strong>Typical Insurer Billing Invoice Report</strong> which tells you the normal type of HCFA-1500 form that is created for insurance invoicing&#8230; either paper HCFA-1500 form or Electronic (via clearinghouse typically). You will also see the typical patient invoice that is used by your office, however this can be set per patient Claim (Claim Wizard page 1). <strong>To generate invoices use the &#8220;Billing Invoice Wizard&#8221; from patient tab.</strong> In step-3 you can override or select the invoice type (mostly patient or hcfa). Consult with your Doctor or other staff as to the regularity which they invoice &amp; submit invoices.</p>

This would be required if you ran out of space on your database (XP SQL Engine has a 2GB file size limit) or if your previous server was Vista/7 but your new server is XP (which is not recommended).<span id="more-1382"></span></p>
<p>Vista/7 on your ChiroPulse Server PC (or stand-alone) uses an updated Microsoft SQL Data engine than the typical XP install. When reinstalling ChiroPulse on the XP SERVER with a Vista/7 database you must follow the Vista/Windows7 installation routines (download). This is necessary since your database is in this Microsoft SQL format (XP installs the Microsoft SQL 2000 MSDE Engine and this cannot read the database from SQL Express 2005).</p>
<p><strong>important note</strong> &#8211; if you are running Windows XP and have Microsoft MSDE2000 installed you should backup your ChiroPulsedbMaster database then use Control Panel&#8230; Add/Remove Programs to remove &#8220;<strong>Microsoft SQL Desktop Engine (CHIROPULSE)</strong>&#8221; prior to installing SQL2005.</p>
<h2><span style="font-size: small;">Step 1.</span></h2>
<p>When installing the Vista/7 Server Microsoft Database Engine you must have <strong>Microsoft dotnet v2</strong> installed on your PC. This is installed with optional Windows updates or you can download the file <strong>dotnetfx.exe</strong> and install that first.</p>

<li>Right click over the ChiroServerTools &amp; Backup icon and in the popup menu select <strong>properties</strong>. Now click &#8220;<strong>Compatibility</strong>&#8221; tab and check &#8220;<strong>Run this program as an administrator</strong>&#8220;.</li>
<li>Now restart ChiroServer Tools &amp; Backup and retry the ZIP backup.This is a permission/ownership issue regarding creation of the backup file. Running ChiroServer Tools &amp; backup as a windows administrator allows it to work as intended.</li>
